Maldonado has some sort of gearbox failure toward the ends of the session.

All of the smoke was because of a gearbox failure? I doubt it, but we'll find out once the team's press release comes out.

What actually happened? Double yellows mean someone is on track no? What exactly did Lewis and Sergio do?

There were waved yellow flags whilst a car was being cleared, however the FIAs system was showing a simultaneous green light and the flashing yellow light was removed from the cars steering wheel. The timing screen also showed that the yellow flags had been removed from the track.

Because there were still manually waved flags being shown they've both been given a penalty.

For Hamilton's offense the car had actually already been cleared....

Neither driver overtook another or did anything dangerous, but they both activated DRS and set fastest sector times. (I believe).

Edit: Apparently Perez's offence was 2 minutes earlier so was whilst the flags were being displayed.

I agree it's harsh that they should be penalized for something that happened in FP; without any meaningful impact on their race and/or qualifying results - but I guess it's more towards health and safety though. Like what would've happened if Lewis' brakes had gone putty as he entered the section...

Agree - but when you've got a bright green light shining at you saying that the zone is clear, picking out the waved flags next to it when you're concentrating on all the other things going on in the cockpit could be a little tricky.

Is it really different in this instance to things like speeding in the pit lane? Given some of the things that other drivers have managed to get away with this year it does seem particularly harsh. Wouldn't a fine have been better? Yet again it would appear that the FIA are unable of being consistent.

The only thing that is a positive is that at least the championship is over now so that the FIA aren't meddling in that. It does probably mean that we're in for another Vettel cruise snooze fest on Sunday though, at the front at least.

Actually this reminds me of the Schumacher in Monaco incident - the FIA messed up the displays, the driver gets penalised. Didn't like it then, don't like it now.
