Has a safety car ever crashed or spun off?
I don't think so, but Grand Prix cars have crashed into the safety and medical car.

Yeah idk about f1 but I’ve seen a c-8 corvette in another racing series crash into the wall...
That was a C7.
C8's just started being used this year in some series.

Possibly, but it was a super short space of time to make that call, less than a 60 seconds window, it's easy to look back now and make the obvious call.
They had a whole lap to decide. When they saw Max enter the pit lane, Lewis was probably just through turn 1. They probably had 1min 25sec or so. So its not a problem. They probably should have pit.

So when Max pitted, Lewis had 2 whole laps to go.
Can you share something that shows this? The live broadcast didn't show it that way. Lewis was ahead of Max by 20 seconds or so? He would have already been 20 seconds around the lap when Max pitted, so it's less than a whole lap to decide surely?
Lewis was about 15 seconds in front of Max, and as mentioned, the pit entry is a good 10 seconds before the end of the lap, so Lewis would have been roughly going through Turn 1. With laps around 90 seconds. Thats plenty of time. Well over a minute. I am more focused on the live timing , rather than the broadcast on TV. It gives you a much better picture as to what is actually going on.

Bottas lasted 38/39 laps!! so close
Yeah, but I thought those estimates were supposed to have a few laps of safety margin.

And didn't they also have several laps behind the safety car?

Kvyat lost his tyre much sooner.
He did, but again could have been something...Grosjean did 36 laps on new mediums starting with a full tank of gas. Maybe the hard just couldn`t handle 40 laps or so!

Horner stated Max`s tyre had slits in it, it wouldn`t be the first time a load of carbon fibre dumped on the track caused punctures!

The tyres are also meant to run out of grip before they fail, Lewis said his tyres felt good enough to keep pushing.

On Mercedes decision to not pit, you would have thought they would have told Lewis, " you are over 30 seconds ahead of Max. Drop your lap time by 10 seconds a lap, tyres are critical"

After today’s tyre failures will Pirelli stick with C2 C3 C4 for next weekend? Bit of a headache.

The team told Lewis not to try for fastest lap, And he said the tires felt fine right up until the very end. They were under the number of laps pirelli said the hards could do. not to mention, they spent several laps behind the safety car so should have been able to go even further.

That leaves us with a few possibilities. There could be a flaw with the pirelli tires (possible, think 2013). Both pirelli and the teams estimates were horribly wrong (unlikely). The punctures were caused by debris from Kimi's incident (most likely).

They did that, and he did slow considerably, it didn't work, probably because it was as a result of debris. Ironically, if he had slowed up any more and still had the puncture, he probably wouldn't have had enough time to limp over the line.

We can hindsight the crap out of this, but they walked away with a win today and they will learn from what happened.

Yeah both Bottas and Sainz complained about some vibrations which were not related to flat spotting during their last stint. Then both had a tire failure...

I would think that punctures were caused by debris and a worn tyre with little rubber will be more prone to punctures than a fresh tyre.

If the tyre was 100% worn we would start to see the canvas and not only would the drivers have no grip but the team would see the massive loss in tyre temperature from their sensors.
